Requesting input on successful sliding scales/fee for services programs

The Standing Committee on the Delivery of Legal Services of the State Bar of California implementing the moderate means suggestions from the newly titled report, "Action Plan for Justice," Appendix 1, Section F, Recommendations 25-27.

We are in the process of identifying organizations and individuals statewide and nationally who have successful sliding scales/fee for services programs, practices and models to further explore.
We are looking for what works, what doesn't work, and how to replicate a successful program, as well as program diversity: practice areas, delivery models, geography (rural v. urban).
