Department of Social Services Seeks Comments on Draft IHSS Forms by 5/4/2010

SACRAMENTO, CALIF (CDCAN) [Updated 04/22/2010 07:16 PM (Pacific Time)] - The Department of Social Services released for public review and comments a revised draft dealing with In-Home Supportive Services (IHSS) provider (worker) enrollment forms. All persons receiving IHSS and all persons who want to work as an IHSS provider (worker) would be required to fill out the forms once final versions are approved and released by the Department of Social Services.

 

 

Below you will find links for the following draft forms:

· 4 page pdf file of the revised draft SOC 426 – This 4 page draft form is for IHSS workers (providers) to fill out.  Please click here: www.calegaladvocates.org/library/attachment.170553

 

· 1 page pdf file of the revised draft SOC 426A – This one page draft form is for persons receiving IHSS (recipient) who would use this form – once the final version is released – to designate and provide information on who their IHSS worker is.   Please click here: www.calegaladvocates.org/library/attachment.170552

The Department of Social Services is requesting that persons interested in providing comments about the revised draft forms should send them by May 4, 2010, Tuesday afternoon, by 5:00 PM via email to:
